Logistics and Procurement

1.0 PURPOSE:

The purpose of this procedure is to establish a policy for the receipt and distribution of surplus medical supplies from the United States Army Medical Materiel Agency (USAMMA) located at the Defense Depot Ogden, Utah (DDO).

2.0 REFERENCES & DEFINITIONS:

2.1. USAMMA Memorandum dated 9 October 1998 with the subject: Medical Supplies Received from USAMMA: This memorandum confirms the sponsorship by the Veterans Administration Medical Center for the sole purpose of allowing the Association of Valley Response Teams (AVERT) to receive and distribute surplus medical supplies for the exclusive use of Community Emergency Response Teams (CERT) in Salt Lake County and throughout other counties in the state. AVERT or any team, organization or activities receiving these supplies are specifically prohibited from selling, hand-receipting or otherwise allowing these supplies to come under the control of any other activity outside of the CERT Program. This does not preclude the lawful redistribution of resources by an incident commander during a disaster or emergency.

2.2. Guidelines for Successful CERT Programs in Utah dated August 15, 1996: This guideline drafted by the State Division of Comprehensive Emergency Management (CEM) outlines the policies and procedures for successful CERT programs in the State of Utah. It provides specific guidance on CERT training.

2.3. AVERT Policies & Procedures #8000 DRAFT, Team Mobilization Checklists: This Standard Operating Procedure (SOP) provides an abbreviated checklist for CERT members and identifies team functional responsibilities.

2.4. Organization: Any subdivision of the AVERT corporation to include member teams, community or district representatives, committees or other structured grouping.

2.5. Grid Leader: A person who coordinates the training and organization of one or more Teams within a particular grid.

2.6. Team Leader: A person who coordinates the training, resources or organization of team members within any particular neighborhood or defined service area.

2.7. Store(s): The physical location or warehouse where USAMMA Surplus Supplies are held prior to issue to requesting qualified teams or Program Managers. This location may also serve as the distribution center and is controlled by the AVERT Board of Directors and the Procurement Committee.

2.8. Issuing Agent(s): Members of the Procurement Committee or CERT Program Manager distributing supplies.

2.9. Active Member Roster: A list of all CERT trained Team Members that have expressed a desire to remain active in the CERT program through regular telephone contact or attendance at team meetings or both.

2.10. Program Manager: Any person who has satisfactorily completed at FEMA-approved CERT Train-the-Trainer course through the State Division of Comprehensive Emergency Management for the purpose of coordinating training in his or her community.

3.0 POLICY OF OPERATIONS:

3.1. Medical supplies from worldwide military sources are shipped to USAMMA for reutilization, sale, destruction or disposal.

3.2. AVERT has been authorized to receive a portion of these supplies for the exclusive use of CERT programs in Salt Lake County and other emergency response teams statewide.

3.3. These supplies must be used only for approved disaster response or emergency training activities only. Acceptance of these supplies certifies that the issued equipment will not be used for any unauthorized purpose.

3.4 Approved CERT activities would include training, drills, search and rescue operations, and during an actual disaster or extraordinary event.

3.5. Supplies will not be sold, bartered, or donated to any organization or individual not engaged in disaster response or emergency response training.

4.0 AVERT/USAMMA PROCEDURE:

4.1. Supplies are procured from USAMMA by an authorized person from AVERT contacting Ken Gronwald (801-399-6005) to determine the availability of supplies and establish a convenient time to pick them up.

      4.1.1. No supplies (i.e. SHARPS, surgical instruments, etc.) will be issued to AVERT which are not appropriate for CERT team use.

4.2. Supplies which are appropriate for CERT use are generally held at the USAMMA warehouse (Building 3C) for up to two weeks awaiting issue to AVERT or to the Weber County Emergency Management Office for temporary storage subsequent to relocation to a CERT distribution facility.

4.3. AVERT, through the Procurement Committee, shall maintain a running inventory database of surplus medical supplies available for issue (see Attachment A), a "Basis of Issue" to determine quantity of items being issued (see Attachment B) which identifies the division of USAMMA supplies between the CERT Coordinator at the State Division of Comprehensive Emergency Management for state-wide distribution and Salt Lake County CERT programs.

      4.3.1. An updated inventory of surplus medical supplies and basis of issue will be posted on the Equipment Locations page of the AVERT Web Site and mailed to active Team Leaders on record prior to any planned distribution date.

4.4. AVERT, through the Procurement Committee, shall maintain sufficient blank "Hand Receipt" forms at the distribution facility on the distribution date to facilitate issue of supplies to Grid and Team Leaders, or their designated signatories, and to program managers.

4.5. The Issuing Agent(s) shall insure that Form 1500 HR (see attachment C) is properly completed by the receiving organization.

4.6 The Issuing Agent(s) shall obtain and transmit to the Record Keeping Committee the following information:

      4.6.1. The exact physical locations of the receiving Team's Cache and assembly area.

      4.6.2. The names and contact phone numbers of all persons having keys or other access to the receiving team's cache.

4.7. Teams are encouraged to maintain a running inventory list of all equipment within their cache site and provide such list to the AVERT Procurement Committee.

5.0 GRID OR TEAM LEADER PROCEDURE:

5.1. The following eligibility requirements must be met before any AVERT/USAMMA supplies may be issued to a team for it's cache:

      5.1.1. The Requesting Team must be organized according to FEMA's CERT standards (officers for each incident command position or function) and provide the issuing agent with a copy of the team's active member roster.

      5.1.2. The Cache location must be secured against theft and environmental exposure. Sheds and containers should be secured with locks provided by the Salt Lake County Fire Department or the local Municipal Fire Department. This is to insure availability of CERT supplies to the Fire Departments in times of localized small scale events. All items used by the local fire department will be replaced by comparable items of similar quality and cost.

      5.1.3. Cache supplies must be available during a disaster. Basement or garage caches are not recommended locations because property owners cannot guarantee availability without compromising security of their personal property.

      5.1.4. A running inventory of cache supplies expended must be maintained within the cache itself. Form 1500 HR (Attachment C) may be used when issuing supplies from the cache for training or response activities in lieu of a running inventory. Copies of the running inventory or 1500 HR Forms must be brought to the issuing agent when requesting resupply from AVERT distributions.

5.2. When the Grid or Team Leader has determined their team meets the requirements of paragraph 5.1 above, he or she will examine the list of available supplies. If desired supplies are available, the Leader will examine the basis of issue document and the schedule of distributions. These three documents will either be mailed to active team leaders or posted on the Equipment Location page of the AVERT Web Site prior to the scheduled distribution date.

5.3. The Team Leader will copy the active member roster and the cache inventory list (or 1500 HR forms) and transmit the copies to the team representative who is to receive the distribution from the issuing agent. A document listing the exact location of the team's cache and the names and contact phone numbers of all team members that have access to the cache must also be provided to the team's representative.

5.4. The Team Representative will provide suitable transportation and arrive at the distribution site at the scheduled time. The documents of paragraph 5.3 above will be presented to the Issuing Agent(s). The representative will obtain a blank form 1500 HR and record the quantity, unit of issue, and description as it is issued. The representative will sign the form and return it to the Issuing Agent. A photocopy of the completed form may be provided if desired.

5.5. The Team Representative will transport the supplies directly to the Team's Cache and add new supplies to the cache inventory as they are transferred.

5.6. Grid Leaders or Program Managers who are obtaining supplies for reissue to individual teams must first satisfy Section 5 requirements in order to obtain the supplies from AVERT. When Grid Leaders reissue the supplies, they must act as the Issuing Agent in conformance to Section 4 and insure that receiving teams adhere to Section 5 of this procedure.
